How do we obtain a disability certificate?

I want to know how do we apply for a disability certificate? Are there some tests to be undergone? I want to know if we can get 2 disabilities on 1 certificate . i have seen only visual disability but not hearing disability.

 

Is is that these certificates can be obtained only for Specific caste people or anyone can obtain it?
